how to make a pitch bend?

I have 4 knob at the 'game port' to use with Rejoice, the software translates the signals from the joystick to Midi: CC0 0-127 (modulation wheel) but the pitch bend does not work in the same way, as I can do to make Joystick -> Rejoice-> Bome T -> Pitch bend of VSTi or DXi.
